Health History Form
If you are new to Simple Wellness, we kindly ask that you complete the health history form included in your confirmation email prior to your first appointment. For future visits, please review and update the form as needed. A link to the form will also be provided in your appointment reminder emails.
Creating a Quiet, Peaceful Environment
We strive to maintain a calm and serene space for all our clients. Please be mindful of others by using a quiet, “library” voice while in our office.
Arrival Time
Please plan to arrive 10 minutes early for your appointment. If you arrive earlier than 10 minutes, we kindly ask that you wait in your vehicle to help us limit the number of guests in the lobby and maintain a peaceful atmosphere.
Communication & Comfort
Before your session begins, please share any recent injuries, concerns, or specific areas of focus with your therapist. This allows us to tailor the session to your needs, ensure your comfort, and remove any uncertainty about your treatment.
Gratuity & Feedback
While tipping is not required, it is always appreciated. We also welcome your feedback—your written reviews help us grow and continue to provide outstanding service.

Cancellation & Rescheduling Policy
Time is valuable—for both our clients and our staff. We require 24-hour notice to cancel or reschedule any appointment.
New clients are asked to pre-pay at the time of booking, which helps uphold our commitment to this policy.
Appointments canceled or rescheduled with less than 24-hour notice, or missed entirely, will be charged the full service fee. Gift certificates will be redeemed in full in these cases.
Refunds are not issued for missed appointments or no-shows.
Existing clients who cancel or reschedule without proper notice will receive an invoice via email, which is expected to be paid on the same day.
Our online booking system is available 24/7 and allows you to reschedule or cancel your appointment outside of the 24-hour window. If you experience any issues accessing your account, please don’t hesitate to contact us—we’re here to help.
