The History of You
(a.k.a Health History Form)
For many, sharing specifics of our health may feel very personal. You may question why my massage therapist needs to know what medications (OTC or prescribed) you take, surgeries, diagnoses, or current treatments you are going through. As licensed massage therapists, we require this information to ensure that the bodywork you receive benefits your well-being and does not cause harm in any way. All information medical and otherwise is kept confidential
Cancellation policies:
If you are a late cancellation (less than 24 hours notice) or are a no-show for your appointment, your payment is forfeited. You do have the option to gift your time to someone else. If you happen to show up late, the Massage Therapist will work with the time remaining.
If a Massage Therapist cancels the appointment less than 24 hours in advance (if they are sick, for example) we will give the client the option to book the same day with any available massage therapist or we will financially compensate the client for their appointment. If the client chooses to be booked with a different massage therapist, no compensation will be given and we will consider the matter settled.
A Friendly Reminder About Gift Certificates
We’d like to remind you that our gift certificates never expire. However, after 12 months of inactivity, they will enter a dormant status. Once dormant, a maintenance fee of $2 per month will be deducted from the balance.
For example, if a $200 gift certificate was purchased in 12/22 and remained unused for 12 months, the current value(4/25) would be approximately $78.
We make every effort to inform purchasers of this policy at the time of purchase, and a reminder is also printed at the bottom of each gift certificate for your convenience.
